Candidates may nominate themselves or be nominated.  Nominees or nominators will be responsible for supplying the following required application material.  All material is to be in a font size of not less than 12 points and provided electronically in one PDF file by close of business 15 March 2013.

·         Summary of accomplishments and their relevance to the selection criteria consisting of up to three pages.

·         Two testimonials that support the nominee’s accomplishments.

·         A short CV (at most 5 pages).

·         At most up to 2 additional evidentiary examples of nominees work.  (OPTIONAL)
